It is with great sadness that I inform you of the Line of Duty Death of Lieutenant/EMT Kevin P. Ward of Chicago, Local 2.  Brother Ward passed away on Monday, August 28th, from injuries sustained while battling a house fire on August 11th. Brother Ward was 58 years old.

By orders of AFFI President Chuck Sullivan, the AFFI Honor Guard is activated.

Brother Ward’s services are as follows:

Visitation: Wednesday, September 13th, 2023, from 0900-1100hrs, at Fourth Presbyterian Church- 126 E. Chestnut St., Chicago, IL 60611. There will be a firefighter walk-through at 1030hrs. Honor Guard members should arrive no later than 1000hrs.

Service: Wednesday, September 13th, 2023, at 1100hrs, at Fourth Presbyterian Church- 126 E. Chestnut St., Chicago, IL 60611

Interment: Private.

It is with great sadness that I inform you of the Line of Duty Death of Firefighter/EMT Jermaine A. Pelt  of Chicago, Local 2.  Brother Pelt passed away Tuesday, April 4th while battling a house fire. Brother Pelt was 49 years old and is survived by two children.

By orders of AFFI President Chuck Sullivan, the AFFI Honor Guard is activated. 

Brother Pelt’s services are as follows:

Visitation: Thursday, April 13th, 2023, from 1500hrs to 2200hrs, at Blake-Lamb Funeral Home 4727 W 103rd St Oak Lawn IL  60453. There will be a firefighter walk-through at 1900hrs. Honor Guard members should arrive no later than 1830hrs.

Service: Friday, April 14th, 2023, at 1100hrs, at House of Hope 752 E 114th St. Chicago, IL 60628.

Interment: Friday, April 14th, 2023, following the service, at Lincoln Cemetery 12300 S. Kedzie Ave. Blue Island, IL  60406.

I know most of you are already aware of the tragedies that struck Chicago, Local 2 this week. Two brothers gone in two days, on separate incidents. We have the arrangements for Lieutenant/EMT Jan Tchoryk. As soon as we get the information for Firefighter Jermain Pelt, I will forward it.

It is with great sadness that I inform you of the Line of Duty Death of Lieutenant Jan M. Tchoryk of Chicago, Local 2.  Brother Tchoryk passed away Wednesday, April 5th while battling a high-rise fire. Brother Tchoryk was 55 years old and is survived by his wife and three adult children.

By orders of AFFI President Chuck Sullivan, the AFFI Honor Guard is activated. 

Brother Tchoryk’s services are as follows:

Visitation: Wednesday, April 12th, 2023, from 1500hrs to 2100hrs, at Cumberland Chapels- 8300 W. Lawrence Ave., Norridge, IL 60706. There will be a firefighter walk-through at 1930hrs. Honor Guard members should arrive no later than 1900hrs.

Service: Thursday, April 13th, 2023, at 1000hrs, at St. Joseph Ukrainian Catholic Church- 5000 N. Cumberland Ave., Chicago, IL 60656.

Interment: Thursday, April 13th, 2023, following the service, at St. Nicholas Ukrainian Catholic Cemetery- 8851 W. Higgins Rd., Chicago, IL 60631.

It is with great sadness that I inform you of the Line of Duty Death of Engineer Jami Lusher of Pekin, Local 524. Brother Lusher passed away Monday, October 17th from colon cancer. Brother Lusher was 49 years old. He is survived by his wife and two children.

By orders of AFFI President Chuck Sullivan, the AFFI Honor Guard is activated. 

Although this is an activation, the family has asked that the services be of a more private nature, with participation being limited to local departments and first responders. For this reason, we are asking that only 3rd District membersof the AFFI Honor Guard attend.

Brother Lusher’s services are as follows:

Visitation: Friday, October 21st, 2022, from 1600hrs to 1900hrs at Henderson Funeral Home- 2131 Velde Dr., Pekin, IL 61554. There will be a uniformed personnel walk through at 1800hrs. 3rd District Honor Guard members should arrive no later than 1700hrs.

Funeral: Saturday, October 22nd, 2022, at 1300hrs at Henderson Funeral Home- 2131 Velde Dr., Pekin, IL 61554. The family has asked that the service be limited to local surrounding departments.

Please do whatever you can to attend as requested and honor the family’s wishes. Make sure you have a plain black mask. I don’t know if there will be any restrictions in place, but everyone should be prepared. I know earlier in the week I sent the notice of the passing of Brother Truevillian from Chicago, L2. Brother Truevillian, 55, passed away on December 17th from COVID-19. I know that in that notice, you were all told his services were incomplete and that a memorial service would take place at a later date.

Since that notice, we have received additional information. On the orders of President Sullivan, the AFFI Honor Guard has been activated. I know this is short notice, but I hope you will all do whatever you can to help us honor Brother Truevillian, his family, and his Local.

His services are as follow:

VISITATION: Sunday, December 27th, 2020 at Russo Hillside Chapels- 4500 Roosevelt Rd. Hillside, IL 60162 from 1500-2000hrs. There will be a fire department walk-thru at 1900hrs. Honor Guard members are asked to arrive no later than 1830hrs. Please make sure you have a plain black mask to wear. We ask that you remain outside until the walk-thru, and maintain appropriate social distance.

SERVICE: Monday, December 28th, 2020 at Russo Hillside Chapels- 4500 Roosevelt Rd. Hillside, IL 60162 at 1100hrs. Services will be streamed at: vimeo.com/494181537

INTERMENT: Monday, December 28th, 2020 at Oakridge-Glen Oak Cemetery- 4301 W. Roosevelt Rd. Hillside, IL 60162 immediately following the service.

Brothers and Sisters,

With great sadness, I must inform you that Brother Robert Truevillian, Chicago Firefighters Local 2, passed away on December 17, 2020 due to COVID-19. Brother Truevillian, 55, was a 20 year member and Paramedic In Charge assigned to Ambulance 71.

Although this is considered a Line of Duty Death, there is no action required of the AFFI Honor Guard at this time. Due to the global pandemic, arrangements and services are incomplete. According to Chicago Fire Commissioner Richard Ford, CFD will be planning a memorial service in the future and details will be provided then.

Please keep Brother Truevillian, his, family, and Local 2 in your thoughts and prayers as we continue to navigate our way through this unprecedented time.

By orders of AFFI President Chuck Sullivan, the AFFI Honor Guard has been activated for the line of duty death of South Holland Local #4109 Firefighter/Paramedic Dylan Cunningham. Brother Cunningham passed on Wednesday, September 30, 2020 after suffering a medical emergency during a training exercise.  Brother Cunningham was a member of the AFFI Honor Guard, having been inducted in 2019.

Please bring local flags, pikes and axes for use at the services. As previously directed, all Honor Guard members should wear a plain black mask at all times.  Additional precautions for each facility will be discussed prior to the services.

Arrangements for Dylan Cunningham are as follow:

Visitation:  Tuesday October 6 from 1pm – 8pm at Smits Funeral Home located at 2121 Pleasant Springs Lane in Dyer, IN 46311

Firefighter Walk-Thru:  Line up at 6:00pm, walk thru at 7pm

Funeral Service: Wednesday, October 7 at 11:00am at Smits Funeral Home. Honor Guard members should assemble no later than 10:00am.

Procession to South Holland Station 43 then to St. John Catholic Church in Glenwood.
