To All Fire Departments/Districts in Illinois:

The Annual Illinois Fallen Firefighter Memorial and Firefighting Medal of Honor Awards Ceremony will be held on May 12, 2026, in Springfield Illinois.  Attached, you will find the application for the Medal of Honor.  We ask for you to complete this application and forward it to the Office of the State Fire Marshal (OSFM).  Please be advised the Committee will review the recommendations and select applicants for the Medal of Honor, or Medal of Valor, Company Unit Citations, Firefighter Excellence, and letters of recognition.

We, as a Committee, are also asking for your help with Line Of Duty Deaths (LODDs).  If your department/district has experienced a LODD and you are aware of eligible dependents that might qualify for educational support at the college level, please contact me.  We may be able to assist the family with college education financial support.
